Principal Care Management (PCM) services represent a new Centers for Medicare and Medicaid Services (CMS) initiative for 2020.  PCM services are intended to compensate specialists and primary care providers for the non-face-to-face care they provide to Medicare patients with one high-risk or complex condition.  PCM services are similar in many ways to Chronic Care Management (CCM) services, although there are important differences.  This white paper provides a detailed overview of PCM requirements based on information provided by CMS in the 2020 Physician Fee Schedule Final Rule.

What are the PCM codes?

There are two billable PCM service codes:

  • G2064: This code may be submitted when the qualified health care practitioner (QHCP) (e.g., a physician, nurse practitioner, physician assistant, certified nurse specialists, etc.) personally spends 30 minute or more per month providing non-face-to-face management services in a calendar month.
