PCM is best-suited for specialties that invest considerable time managing patients via non-face-to-face services. In order to be eligible for PCM services patients clinicians need to manage or comanage a single high-risk or complex condition. All specialties of medicine and surgery are eligible to provide these services. PCM is applicable to primary care clinicians for patients that have a single high-risk or complex condition.
