PCM for Family Medicine

Family physicians care for patients with a single high-risk or complex condition. These patients may be eligible for Principal Care Management (PCM) services. PCM services may complement Chronic Care Management (CCM) services as they can be offered simultaneously to patients in the same practice.

Examples of conditions that may qualify for PCM services include:

Poorly controlled hypertension
Diabetes with complications
COPD

Cerebrovascular disease
Congestive heart failure
Coronary artery disease
Major depressive episode
Gastric ulcer disease
Hepatitis C

Lupus
