PCM for Cardiology

Principal Care Management (PCM) is well-suited for cardiology as cardiologists often manage or co-manage patients with high-risk or complex conditions.  One of the first steps a cardiology practice should take is to identify how many patient may qualify and benefit from PCM services. 

Examples of conditions that may qualify for PCM include: 

  • Heart Failure
  • Coronary artery disease
  • Post-myocardial infarction complications
  • Ischemic cardiomyopathy
  • Kyphoscoliotic heart disease
  • Pulmonary hypertension
  • Chronic pulmonary embolism
  • Dilated cardiomyopathy
  • Alcoholic cardiomyopathy

  • Atrioventricular block
  • Ventricular arrhythmia
  • Atrial fibrillation
  • Sick sinus syndrome
  • Labile hypertension
